Arrival
New patients should arrive 15-20 minutes before appointment time to fill out medical history and other necessary forms. You may also fill out these forms ahead of time by printing them off this Web site.
If you have any changes in insurance address or other information, please arrive early. Patients who do so assist in maintaining patient flow and provider schedules.
What to bring
Bring your insurance card and photo identification so the staff can make a copy of them.
Payment
Co-payments are due at the time of service and will be collected during the check-in process. This is required by your insurance contract. Our billing office will make every attempt to assist you with your account. If you have questions about your account, please call our office. We accept cash, checks, Mastercard and Visa.
Cancellations
If you find that it is impossible to keep your appointment, we ask that you inform us at least 24 hours in advance. This courtesy allows us to be of service to other patients.
